Ice vs. Heat: The #1 Question We Get Asked at Epic PT
If we had a dollar for every time someone asked, “Should I use ice or heat?” we’d have an ice machine and a sauna in every treatment room 😄.
It’s one of the most common questions we get at Epic Physical Therapy, and it’s a good one. Both ice and heat can help with pain, but using the right one at the right time makes all the difference.

🧊 ICE — Calm It Down
Best for:
- Recent injuries (within the first 24–72 hours)
- Swelling, inflammation, or bruising
- Pain that feels sharp, warm, or throbbing
Ice (also known as cryotherapy) helps by reducing blood flow to the injured area, which limits inflammation and numbs pain.
What research says: A 2022 Journal of Athletic Training review found that short bouts of ice (10–20 minutes) can effectively decrease pain and swelling in acute injuries. Just remember — too much icing can slow tissue healing, so moderation is key.
💡 PT Tip: Always use a towel barrier between your skin and the ice pack to prevent frostbite or irritation.
🔥 HEAT — Loosen It Up
Best for:
- Chronic pain, stiffness, or muscle tension
- Arthritis flare-ups
- Tight neck, shoulders, or low back
Heat (also called thermotherapy) helps by increasing blood flow and improving tissue flexibility. It can also relax tight muscles and make it easier to stretch or move — which is why we often combine it with mobility work in physical therapy.
What research says: Multiple studies show that heat therapy increases tissue elasticity and reduces muscle tension, especially when paired with movement.
💡 PT Tip: Moist heat (like a warm shower, heating pad, or towel) often penetrates deeper and feels more soothing than dry heat.
