Your MRI Findings May Not Be the Real Reason You’re in Pain
What to Do If Your MRI Shows a Disc Bulge, Meniscus Tear, or Arthritis in Highlands Ranch & South Denver
If you recently searched:
- “MRI shows disc bulge now what?”
- “Do I need surgery for meniscus tear?”
- “Arthritis on MRI but severe pain”
- “Should I see a surgeon after MRI?”
- “Physical therapy instead of surgery near me”
You’re not alone.
At Epic PT in Highlands Ranch and Inverness, we regularly meet patients who walk in holding an MRI report — worried, discouraged, and convinced they’re “broken.”
Here’s something important to understand:
Your MRI findings may not actually be the reason you’re in pain.
Why MRI Findings Don’t Always Match Your Symptoms
MRIs are excellent tools. They show structure. They show tissue. They show anatomy.
But they do NOT measure:
- Muscle timing
- Joint loading patterns
- Strength deficits
- Balance and proprioception
- Nervous system sensitivity
- Movement coordination
Multiple research studies show that many adults — especially over 40 — have:
- Disc bulges
- Herniated discs
- Meniscus tears
- Labral tears
- Rotator cuff tears
- Arthritis
… and have zero pain.
If your MRI says “degeneration” or “tear,” that does not automatically mean surgery is required — or that the finding is even the primary cause of your symptoms.
Common MRI Findings We See in South Denver Patients
1. Disc Bulge or Herniated Disc (Low Back or Neck)
Common searches:
- “Do I need surgery for disc bulge?”
- “Herniated disc treatment without surgery”
- “Best physical therapy for sciatica Highlands Ranch”
Many disc issues respond extremely well to targeted strengthening, movement retraining, and load management — without injections or surgery.
2. Meniscus Tear on MRI
Common searches:
- “Meniscus tear MRI results”
- “Knee surgery or physical therapy?”
- “Can you rehab a meniscus tear?”
Not every meniscus tear needs surgery. Many people improve significantly with conservative physical therapy focused on quad strength, hip control, and knee mechanics.
3. Arthritis on MRI or X-Ray
Common searches:
- “Bone on bone knee pain”
- “Arthritis treatment near me”
- “Physical therapy for arthritis Medicare”
Arthritis on imaging does not automatically equal severe pain. Many active adults and Medicare-aged patients in the South Denver metro area maintain strong, independent lives with the right strength and mobility plan.
So What Does Cause Pain If It’s Not Just the MRI?
Pain is complex.
Often it’s driven by:
- Poor load tolerance (doing too much too fast)
- Weakness in supporting muscle groups
- Movement compensation patterns
- Loss of proprioception (joint awareness)
- Balance deficits
- Deconditioning after injury
- Nervous system sensitization
This is why a thorough orthopedic physical therapy evaluation matters.
At Epic PT, we assess how you move — not just what your scan says.
Should You Get an MRI Before Physical Therapy?
Common searches:
- “Do I need an MRI before PT?”
- “Can I start physical therapy without imaging?”
In many cases, you can start physical therapy without an MRI.
Clinical guidelines often recommend conservative care first for:
- Low back pain
- Knee pain
- Shoulder pain
- Neck pain
Unless there are red flags, starting with physical therapy can reduce unnecessary imaging, injections, and surgery referrals.

When Is Surgery Necessary?
There are absolutely times when surgical consultation is appropriate.
But surgery should be based on:
- Clinical symptoms
- Functional limitations
- Failed conservative care
- Neurological findings
— not imaging alone.
Your MRI is a piece of information.
It is not your identity.
It is not your destiny.
